In the consideration of The Max Hockey School accepting as students in the said Hockey Club. It is agreed that the said Hockey Club is hereby released from any and all claims, damages, or causes of action. It is agreed that all risks, relating to the hockey school are assumed without limitation by the registrant and that this assumption of risk is acknowledged, approved, and agreed to by the registrant as indicated by checking the box below.
